Hallux (Big Toe) Limitus & Rigidus Series
Part 3 – Recommendations on Custom Orthotic Designs
For Hallux Limitus/Rigidus, custom orthotics should focus on reducing first MTP joint (Big Toe) motion while maintaining support and comfort.
1. Orthotics for Hallux Limitus (Early Stage)
– Morton’s Extension: A rigid extension under the first metatarsal and hallux (Big Toe) to limit dorsiflexion and reduce stress.
– Carbon Fiber or Polypropylene Plate: Semi-rigid reinforcement to control excessive movement while allowing some flexibility.
– Deep Heel Cup (Orthotic Heel is deep): Provides rearfoot stability and improves weight distribution.
– Medial Arch Support: Helps offload pressure from the first MTP joint.
2. Orthotics for Hallux Rigidus (Advanced Stage)
– Full-Length Rigid Plate: Limits painful joint movement entirely.
– Rocker-Bottom Sole Integration: Can be used in shoes to facilitate forward motion with minimal toe bending.
– Extra Forefoot Cushioning (Poron/EVA Layers): Reduces impact forces on the stiff joint.
– First Met or First Ray Cutout (Optional): In some cases, relieving pressure under the first metatarsal head can help with comfort.
X-ray results can be important too. Common signs of this condition is narrowing of the joint space (early stage is less narrowing than later stages) and Osteophyte (Bone Spur) formations on the Big Toe joint mostly on the Dorsal (on top) side of the joint (this is more common in Hallux Rigidus/later stages). These Bone Spurs can affect the orthotic design since some patients might feel discomfort and pain if the orthotic makes the shoe space small either through the thickness or modifications. Due to this, the Bone Spur areas come in contact with the shoe which makes it painful for the patient.
