Part 4 – Morton’s Extension VS 1st Met Cutout
In our previous post we mentioned Morton’s Extension and 1st Met Cutout, let’s look at them more deeply.
Morton’s Extension is a rigid extension under the first metatarsal and hallux (Big Toe) to limit dorsiflexion and reduce stress.
1st Metatarsal Cutout is when a portion of the orthotic material is removed/cut out around the area of the first metatarsal head to relieve pressure from this area.
So which is better?
The choice between a 1st Metatarsal Cutout and a Morton’s Extension depends on the patient’s condition and needs.
Features of Morton’s Extension:
Purpose & Effect: Restricts 1st Metatarsal dorsiflexion (upwards bending) to reduce pain and joint irritation
Best for: Hallux Rigidus (advanced stage, stiff joint)
Materials: Carbon fiber, polypropylene, or rigid EVA extension
Footwear Compatibility: Requires stiff-soled shoes for best results
Features of 1st Metatarsal Cutout:
Purpose & Effect: Enhances first ray plantarflexion (downwards bending) for better propulsion and helps movement by offloading pressure
Best for: Hallux Limitus (early stage, some mobility remains)
Materials: Polypropylene/EVA with localized cutout.
Footwear Compatibility: Works well with flexible shoes
Summary of which is Better for each condition?
Hallux Limitus (Mild to Moderate Cases) ➡ 1st Met Cutout is preferable since it encourages motion.
Hallux Rigidus (Advanced Cases) ➡ Morton’s Extension is better to restrict painful movement.
