A 27-year-old male runner presented with lateral knee pain for the past two months, especially during long runs. Pain worsened on downhill routes, and he reported occasional swelling.
Clinical Background
The patient runs 25–30 km per week and recently increased mileage. Tightness in hip abductors and quadriceps was noted.
Symptoms
– Sharp lateral knee pain during running
– Worsens with long or downhill runs
– Occasional swelling
– Tightness in hip and thigh
– Relief with rest
Clinical Findings
Excessive foot pronation and weak hip abductors contributed to lateral knee stress. No joint restrictions.

Diagnosis
Iliotibial (IT) Band Syndrome linked to Biomechanical Imbalance (based on lateral knee pain, overpronation, and muscle tightness).
Management Plan
– Custom Orthotics to correct pronation
– Supportive footwear
– Stretching and strengthening of hip and thigh muscles
– Activity modification and gradual load management
– Follow-up to assess symptom resolution
