A 52-year-old female presented with pain and swelling at the base of her right big toe for the past six months. She reported difficulty wearing shoes and discomfort when walking long distances.
Clinical Background
The patient has a history of narrow footwear use and she works in retail, requiring prolonged standing.
Symptoms
– Pain and swelling at first metatarsophalangeal joint
– Difficulty fitting shoes
– Redness and tenderness
– Discomfort during walking
– Symptoms worsen with activity
Clinical Findings
Examination revealed lateral deviation of the hallux and medial prominence. There was tenderness over the bunion with mild inflammation.
Answers
Diagnosis
Bunion (Hallux Valgus) (based on toe deviation, pain location, and gait assessment).
Management Plan
– Custom Orthotics to offload the bunion and support the first ray
– Supportive wide-toe footwear
– Activity modification to reduce forefoot pressure
– Education on footwear choices
– Follow-up to monitor symptoms and adjust orthotics
