A 28-year-old female presented with aching pain along the inner aspect of both feet for the past six months. She reported increased discomfort after long shifts as a nurse, especially when standing or walking for extended periods. She also noticed mild knee discomfort by the end of the day. Symptoms developed gradually with no specific injury.
Clinical Background
The patient works full-time in a hospital setting, requiring prolonged standing and walking. She has no significant medical history and no previous foot injuries. She typically wears standard running shoes without additional support.
Symptoms
– Aching pain along the medial arch of both feet
– Foot fatigue after prolonged standing
– Mild swelling along the inner ankle
– Occasional knee discomfort
– Symptoms worsen by end of day
Clinical Findings
Examination revealed reduced medial arch height during weight-bearing and excessive pronation during gait. The arch reappeared when non-weight-bearing. There was mild tenderness along the posterior tibial tendon with no joint restrictions.
Answers
Diagnosis
Flexible Flat Foot (Pes Planus) with Overpronation (based on excessive pronation and decreased arch height during weightbearing but arch reformation when non-weightbearing).
Management Plan
– Prescription of Custom Orthotics to support the medial arch and control pronation
– Supportive footwear with firm heel counter and stability features
– Physiotherapy to strengthen intrinsic foot muscles and improve mechanics
– Education on proper footwear and activity modification
– Follow-up to monitor symptom improvement and adjust orthotics if needed
