A 46-year-old female presented with sharp, burning pain in the forefoot between her 3rd and 4th toes for the past four months. She reported tingling and numbness in the toes, especially when wearing tighter shoes. Symptoms improved when she removed her shoes.

 

 

Clinical Background

 

The patient works in an office setting and frequently wears narrow dress shoes. She has no significant medical history and no recent trauma.

 

 

Symptoms

 

– Burning pain in the forefoot

– Tingling and numbness in the toes

– Sensation of a pebble in the shoe

– Pain worsens in tight footwear

– Relief when removing shoes

 

 

Clinical Findings

 

Examination revealed tenderness between the 3rd and 4th metatarsals. Compression of the forefoot reproduced symptoms. There were no joint restrictions.

Diagnosis

 

Morton’s Neuroma (3rd Interspace) (based on location of pain, neurological symptoms, and reproduction of symptoms with compression).

 

 

Management Plan

 

– Prescription of Custom Orthotics with metatarsal support to relieve nerve pressure

– Footwear modification with wide toe box and cushioned soles

– Education on avoiding tight footwear

– Activity modification to reduce forefoot stress

– Follow-up to monitor symptoms and consider further intervention if needed
