Let’s look at Achilles Tendinitis Case Study!
A 40-year-old male runner presented with stiffness and pain at the back of his right heel for the past three months. Pain was worse in the mornings and after long runs. He reported difficulty with hill sprints and climbing stairs. Symptoms developed gradually without a specific injury.
Clinical Background
The patient runs 20–25 km per week and recently increased the amount of km he ran. He has no prior Achilles injuries but wears standard running shoes with minimal cushioning.
Symptoms
– Pain and stiffness along the Achilles tendon
– Tenderness on palpation
– Swelling in the mid-portion of the tendon
– Discomfort during running or jumping
– Morning stiffness
Clinical Findings
Examination revealed tenderness 4–6 cm above the heel insertion, tight calf muscles, and mild swelling. There were no joint restrictions.
Answers
Diagnosis
Mid-portion Achilles Tendinopathy (based on pain location, tenderness, activity history, and examination findings).
Management Plan
– Prescription of Custom Orthotics to correct mechanics and reduce tendon load
– Supportive shoes with cushioned heels
– Calf stretching and eccentric strengthening exercises
– Activity modification and gradual load progression
– Follow-up to monitor tendon recovery
